Quality of Care Indicators

As a Community Health Center, River Hills reports health care quality measures annually to the federal Department of Health and Human Services, Bureau of Primary Health Care using the Uniform Data System (UDS). Example UDS measures include:


Hypertension Control     

Ensuring that hypertensive patients blood pressure readings are properly/regularly collected and appropriate actions are taken to address high blood pressures.

Childhood Immunizations

Ensuring that children are fully immunized before their 3rd birthday to decrease the chances of them contracting preventable diseases.

Cervical Cancer Screening     

Ensuring that women are properly/regularly screened for cervical cancer in order to increase early detection of any abnormalities.

Diabetes Control     

Ensuring that diabetic patients HbA1c readings are properly/regularly collected and appropriate actions are taken to address high HbA1c readings.

Colorectal Cancer Screening

Ensuring patients are properly/regularly screened for colorectal cancer in order to increase early detection of any abnormalities.
